Two studies will be presented on the topic of organizational factors in patient safety. The first, summarized briefly, uses surveys and administrative (claims) data to examine the relationship of Veterans Health Administration (VA) hospitals� organizational culture and climate to indicators of patient safety. The second, presented in more detail, is an exploratory set of case studies that describes the work of managers in building a culture of patient safety. It compares managers� actions across four facilities with differences in patient safety indicator rates. We found corresponding organization-level differences in how managers go about developing a positive safety culture: for example, actions we categorize as �strategic� (e.g., task-focused) dominated only at the lowest-performing hospital. We develop propositions for future research to test and explain the relationships that we found between organizational characteristics and managers� culture change actions. |
